She fell on a Saturday. By June she was discharged at low risk.

A story from Bayada

F.S. is in her nineties. Two days after her first OneStep walk flagged her as high fall risk, she fell in her bathroom. Her Timed Up & Go slipped from 22.9 to 30.3 seconds. It could have been the start of a decline. It became the start of a recovery.

THE CHALLENGE

Bayada's therapists in six senior living communities needed a way to find the residents who needed therapy before they fell, prove that treatment was working, and keep residents motivated between sessions.

HOW ONESTEP HELPED

Residents walked with a phone in a pocket. OneStep returned a walk score and fall-risk level on the spot at wellness screening days, tracked recovery walk by walk on caseload, and kept measuring passively in between — 3,000 walks from 18 residents that never required a session. F.S. started checking her scores; watching them climb became the reason she did her home exercises.

WHAT CHANGED

F.S.'s TUG came back to 16.3 seconds, her walk score rose from 66 to 80, and she was discharged in June at low fall risk. Another resident went from medium to low risk in three weeks; a third's sharp decline was caught mid-program and reversed with no further falls. Across the pilot, 79% of residents held steady or improved, and one in three residents screened at a wellness day was referred to therapy they weren't yet receiving.

When residents can see their own progress, they do the work. When therapists can see the decline, they catch it.
