J.P. has heart failure, lung disease and stage 3b kidney disease. Any one of them can impact mobility. In the spring, two separate declines were identified on her scheduled walks. Both showed up before anyone would have seen them at a review.

THE CHALLENGE

With a list like hers, the honest question is not how much she can gain. It is how much she can keep. Unfortunately, decline is the expected path. When it arrives slowly, it is easy to accept as the disease doing what it does. The hard part is telling a real decline from a bad day. Then proving the difference after you have treated it.

HOW ONESTEP HELPED

Both declines appeared on routine OneStep walks rather than at a review. Her Berg Balance came back 52 out of 56 in May. Each decline was worked, and then re-tested against her own prior walk. That second step is what made the recovery confirmed rather than assumed.

WHAT CHANGED

Her risk climbed toward Medium-High twice in the spring. Since May it has stayed at Low, four months running. Every walk in that window sits above the norm for her age. Her longest walk on record came in July. 

Four months without losing ground is the care plan working. Measured, not claimed.
