Prestige Rehab provides therapy inside senior-living communities and skilled nursing facilities across the East Coast. It picked OneStep in 2024 because it was the easiest tool to use. Two years later it runs OneStep in 29 communities, records about 1,500 measurements a month, and generates roughly $102 of Remote Therapeutic Monitoring revenue per monitored resident, every month — from walks its clinicians would be measuring anyway.
Fall risk in a population where decline is expected rather than prevented. The Berg and Dynamic Gait Index are clinically sound, but they are limited by what a resident can do on the day and they produce a score, not a picture a family can follow. Prestige wanted something objective enough for clinicians, simple enough to use on every unit every day, concrete enough to explain to a daughter — and, with rehab providers under pressure to do more with less, able to pay for itself.
A resident walks down the hallway with a phone in a pocket and the clinician has gait speed, step symmetry, double support and a fall-risk level before they reach the end of it. Non-ambulatory residents are measured with sit-to-stand and transfer tests, so nobody is left out. OneStep trained Prestige's regional leads once; they onboarded each of the 29 communities with a one-page quick-start guide, and clinicians were measuring on the first day. Every measurement is also an RTM event that OneStep tracks for billing automatically — including patient activity, which the app earns passively by monitoring the resident's movement in the background of their phone.
Prestige now records more than 1,500 active measurements a month — 25,000 since launch. Clinically, 73% of residents measured improved or maintained function and 36% showed a significant reduction in fall risk, in a population where two-thirds of assessed residents are at high or very high fall risk. Financially, Prestige has generated between $27,000 and $48,000 of RTM-eligible revenue every month for the last twelve months — $492,000 in total, about $102 per monitored resident per month, a quarter of it earned passively by the app with no clinician time at all.
